* Premium Membership Level Only. Limited to two visits per year when clinically necessary or at the physician's discretion. House and office calls are provided by your personal physician and/or the physician's designee.
**Elective ClienteleHealth benefit. Additional cost to ClienteleHealth membership fees. Executive Physicals are arranged through & provided by participating ClienteleHealth physicians. Cost of Executive Physical is determined by your ClienteleHealth physician.
